Support and Resources

More Than Medical Care
Book a visit →

Your health is about more than what happens during a clinic visit.

Sometimes getting and staying connected to care means having help accessing medication, understanding insurance, finding transportation, or connecting with resources in your community.

At Core Life Link, we’re here to help you navigate those needs, too.

Case Management

Healthcare can sometimes feel complicated. You shouldn’t have to figure everything out by yourself.

Our case management services can help you navigate your care, understand your next steps, connect with services, coordinate referrals, and identify resources that may be available to you.

Tell us what you need help with, and we’ll work with you to explore your options.

Medication & Insurance Support

Questions about medication access or insurance can make getting care feel overwhelming.

We can help you explore options for accessing medications, navigating insurance coverage, and identifying assistance programs that may be available to you.

Eligibility varies by program, but we’ll help you understand your options and what information may be needed.

Transportation Support

Getting to care shouldn’t have to be another source of stress.

Transportation assistance may be available for eligible patients who need help getting to appointments or accessing care. Talk with us about your needs, and we can help you explore available options.

Health Education

You deserve information about your health that is clear, understandable, and free from judgment.

Our team can answer questions and provide education about HIV, STIs, Hepatitis C, PrEP, PEP, treatment, prevention, sexual health, and other health concerns.

There’s no such thing as a bad question. If something doesn’t make sense, ask us. We want you to feel informed and confident about your care.

Connections to Community Resources

Sometimes the support you need may be outside of the services we provide directly. When that happens, we can help connect you with other programs and organizations.

Depending on your needs and available resources, we may be able to provide referrals for:

  • Behavioral health services
  • Housing resources
  • Employment support
  • Veteran services
  • Transportation resources
  • Insurance or benefits assistance
  • Other healthcare and community services

Services and eligibility may vary, but we’ll help you understand the options that may be available to you.

Not Sure Where to Start?

Start with us.

You don’t need to know the name of a program or exactly what kind of assistance you need before reaching out. Tell us what’s going on, and we’ll help you identify your next step.

Care that goes beyond the clinic.
